About the Opportunity
Ensures accurate completion of LTCF assessments.
Prepares reports, records, statistics and other data as needed or requested.
Participates as a member of the interdisciplinary team in assisting individual clients to meet their assessed needs.
Provides support to clients and families as appropriate.
Collaborates with clients and family/friends to meet their program expectations and needs.
Supports the coordination of new admissions
Supports ongoing education of employees
Ensure all audits and inspections are completed as scheduled
Collaborates with Site Manager and facility team to prepare for licensing inspections
Prepares for and attends various site meetings such as risk management and family conferences
Collaborates with Site Manager and staffing coordinator to ensure site staffing levels are maintained.
About You
About You
In addition to placing high value on continuous improvement, collaboration and accountability, you bring:
Licensed Practical Nurse (LPN) diploma or other relevant education.
Active membership in a professional association.
CPR Certified.
Proven clinical experience in long-term care or working with seniors.
Experience providing clinical support in a team setting while supporting staff development and fostering a collaborative clinical environment.
Experience in training, mentoring, and fostering teamwork while motivating groups.
Expertise in resident care management with a strong commitment to resident-centered care and high-quality standards.
Ability to collaborate effectively with diverse healthcare teams and contribute to clinical operations.
Understanding of human behavior principles and the ability to apply problem-solving skills.
Awareness of how decisions impact the budget and knowledge of budgetary limitations.
Ability to gather data, compile information, and prepare reports.
Knowledge of community programs and support resources.
A passion for the healthcare sector, especially ensuring seniors have access to quality accommodations, services, and care.
Excellent verbal and written communication skills.
Strong computer literacy, including proficiency in word processing, spreadsheet, and database software.
