The Clinical UCP Supervisor will have the expertise to assess, implement and evaluate a wide range of clinical care interventions to ensure the delivery of safe, quality, and ethical care.**Why you’ll love working here:*** Flexible scheduling and autonomy to manage your day* Regular team appreciation events and a strong culture of support* Opportunity to develop long-term therapeutic relationships with patients* Opportunity to practice and develop complex clinical skills* Office located above St. Patrick Subway Station – easy access via TTC* Company-paid mobile devices* Mileage compensation* Education allowance for employees and family* Employee Assistance Program* Reimbursed parking and access to an in-building gym* Comprehensive health and dental benefit plans* RRSP matching* Best-in-class training and development programs, including comprehensive clinical orientation and hands-on skills labs*\*Option to maintain clinical skills through occasional field work.***What you’ll do:*** Lead and support PSW’s and HSW’s to ensure practices comply with all legislated standards of practice and company internal protocols* Respond to and mentor staff, patients/families, and other team members with regular touchpoints about concerns from community* Provide ongoing leadership through regular huddles, face-to-face meetings and one-on-ones.* Initiate patient safety incident investigations and documents to ensure timelines and action plans are met.* Work collaboratively with nursing team members to support clinical documentation and submission requirements according to specific funder reporting guidelines.* Engage patients in collaborative problem-solving by building trust, providing health education, and identifying their care needs, strengths, and goals* Engage staff in problem solving, offering education, coaching, and mentoring in resolution of problems.* Maintain the knowledge, skills, and attitudes to provide safe, efficient, effective service in all areas of practice by keeping abreast of changes in practice setting that affect scope of practice.**What you bring:*** Current RN registration with the College of Nurses of Ontario and entitled to practice without restrictions* Minimum of 5 years’ experience as a practicing Registered Nurse* Comprehensive patient health assessment skills are required* Possess an understanding of the principles of community nursing practice* Proven experience leading and managing a team* Knowledge of clinical documentation standards and practice guidelines is required* Excellent organizational, interpersonal, and teaching skills* Commitment to professional development in clinical practice that is relevant to home health care* Effective critical thinking and problem-solving skills* Must possess a valid driver’s license and access to reliable transportation* Able to provide a current, clear Vulnerable Sector Screening (VSS)* C.N.A.
