Unit Summary

The Community and Population Health Services portfolio supports the advancement of integrated health and social services in hospital and into the community. The Program is responsible for the development and advancement of innovative partnerships with peer, community academic partners and provides expert leadership in the area of pre- and post-acute care transitions.

The Community and Population Health Services portfolio is accountable for HHS’ Population Health, Bundled Care and Alternate Level of Care strategies.

The portfolio provides system-wide support through the Site Administrative Managers, Transitional Care & Discharge Specialists, ALC Navigators, Centralized Care and Transition Team, Hospital to Home, Integrated Comprehensive Care Program and Inpatient Medical Psychiatry Consultation Liaison Teams. The McMaster Family Health Team (McMaster Family Practice, Stonechurch Family Health Centre and Canadian Blood Services) and Behavioural Supports Ontario (BSO) are aligned to the portfolio.

Position Summary

Responsible to the Director of Community & Population Health Services, the Manager has leadership and managerial responsibilities at both the planning and operational level.

The Manager is expected to design, lead, plan and evaluate improvement efforts for programs within the Community & Population Health Services portfolio, while developing and supporting diverse teams of staff responsible for operationalizing the Community & Population Health Services Program’s efforts at HHS.

Key responsibilities for this role include working with the Director to support the on-going development of Population Health Management at HHS. This includes the oversight of teams focused on achieving the Quadruple Aim for specific populations such as: Inpatient Adult Psychiatry Consultation team, Addictions Counselors, HHS’ Hospital to Home (H2H) team, the Centralized Care and Transitions Team (CCaTT), Integrated Comprehensive Care (ICC) COPD and HF.

  • Regulated Health Professional with current certificate to practice

    Masters Degree in related field preferred
    3. Minimum 3 years formal managerial experience in acute hospital setting

    Minimum 5 years experience working with patients with multi morbidity, mental health and addictions, frailty, impacted by social determinants of health and poor health literacy
    5. Demonstrated knowledge of Ontario Health Teams

    Demonstrated knowledge of community partners and integration of care across sectors
    7. Demonstrated ability and knowledge to access and utilize hospital, primary care, home care and community resources

    Demonstrated knowledge of population health management frameworks
    9. Demonstrated evidence of continuous education and life long learning

    Demonstrated commitment to the mission, vision, and values of HHS
    11. Proven leadership abilities and effective communication, facilitation and presentation skills

    Demonstrated ability to facilitate quality improvement teams, apply quality improvement methods and tools, and coach/mentor staff on the same
    13. Skill in supporting and mentoring staff to enhance knowledge, skill, and judgment within their practice

    Demonstrated leadership and coordination skills in a fast-paced environment
    15. Demonstrated creativity and innovation in the delivery of patient care services

    Demonstrated excellence in communication, team-building, and ability to initiate, model and support change in a constructive manner
    17. Demonstrated advanced critical thinking skills

    Demonstrated skills in data collection design, collection of data, writing reports and analysis
    19. Demonstrated use of various data sources (eg.

    Canadian Institute of Health Information (CIHI) data base, Hay Report, Workload, Canadian Inpatient Surveys, Quality of Life Surveys)
    20. Proficiency in use of office support software (Word, Excel, PowerPoint and Access)
