Sinai Health is looking for a Nurse Practitioner to support Wound Care across both Mount Sinai Hospital and Hennick Bridgepoint Hospital campuses.

The Nurse Practitioner (NP) – Wound Care at Sinai Health, is a Registered Nurse in the Extended Class (RN(EC)) who has additional nursing education and experience. The NP Wound Care, will have the competencies to autonomously diagnose, order and interpret diagnostic tests, prescribe pharmaceuticals and perform procedures within their scope of practice.

The NP’s practice is grounded in the values, knowledge and theories of the professional nursing practice emphasizing health promotion and prevention of illness. The NP will function as a resource to patients, families, and health care professionals.

Additionally, the NP will provide leadership in practice and the development of standards, processes and quality initiatives enhancing the care for patients at Sinai Health. The position will involve a combination of clinical, administrative and research time and the breakdown will be determined at the start of the position and negotiated as the needs of the position change (i.e., 75% clinical, 25% admin/research).



As a member of the interprofessional team, the Nurse Practitioner in Wound Care will function as a specialized hospital wide clinical resource, who provides expert clinical consultation (assessment, collaboration, education, interventions), and care planning for complex wounds, ostomies and continence.

In this role you will:


Provide leadership, mentorship, education and support to the nursing, medical and health discipline staff
Lead evidence based practice for our clinical teams, and the development of standards and quality initiatives enhancing safe high quality patient and caregiver centred care
Consult with and/or refer to other health care professionals at any point in the care continuum when the patient’s condition is beyond the NP scope of practice or the individual NP’s competence
Act as a consultant and accept referrals from health care providers and community agencies as indicated
Advocate for patients/families in relation to therapeutic intervention, health care access, the health care system and policy decisions that affect health and quality of life
Engage in evidence-based practice by critically appraising and applying current evidence (including research, best practices and theory) when providing health care services at the individual, organizational and system level
Collaborate in – and lead – research initiative, disseminates findings as well as acts as a resource for others in the development of research projects
Identify, collect data and evaluate the outcomes of NP practice
Support and maintain the philosophy, goals and objectives of the Department of Nursing and the hospital
Provide leadership in in the management of clinical care and in the development and integration of the NP role within the health care system
Contribute to the development of the nursing profession through active involvement and affiliation with other nursing organizations, professional associations and with the community at large
Facilitate knowledge translation in the clinical setting to support care in highly complex and unpredictable situations

Mandatory


Current registration (Extended Class) in good standing with the College of Nurses of Ontario (CNO)
Masters degree in Nursing
Relevant specialty certificate or completion of Wound Care Education programs required e.g.

Wound, Ostomy and Continence CNA Certification; International Interprofessional Wound Care Course (IIWCC-CAN); Wound, Ostomy and Continence Education Program (WOC-EP™); Wound Healing Master’s Program)
Minimum five (5) years recent relevant nursing experience
Minimum of two (2) years in wound care management
Member of NPAO and/or RNAO
Current Basic Cardiac Life Support (BCLS) certification


Preferred

Eligible for cross-appointment to an academic institution

Skills and Knowledge



Evidence of leadership and management skills.
Experience in developing clinical tools such as policies/procedures, clinical protocols, order sets
Experience and interest in conducting, interpreting and applying research to practice
Demonstrated commitment to collaborative practice
Ability to work effectively independently and under stressful conditions
Demonstrated satisfactory work performance and attendance history
Intermediate Microsoft Office skills (i.e. Word, Excel, PowerPoint, Outlook)
Knowledge of statistics and ability to analyse and synthesize fiscal and patient care data
Excellent interpersonal, communication, leadership and organizational skills
Excellent decision making, problem recognition, problem solving skills, negotiation and conflict resolution skills
Commitment to collaborative and interprofessional practice
Knowledge of relevant legislation and regulations
Knowledge of Meditech and CERNER or computerized documentation skills
